At Proactive Health Management, we are committed to ensuring your privacy through best practice standards.  We promise to use your personal information, only as required to complete our medical service, in a responsible and professional manner.

What is Personal Information?

The Office of the Privacy Commissioner of Canada defines Personal Information under PIPEDA as "personal information includes any factual or subjective information, recorded or not, about an identifiable individual. This includes information in any form, such as:

• age, name, ID numbers, income, ethnic origin, or blood type;
• opinions, evaluations, comments, social status, or disciplinary actions; and
• employee files, credit records, loan records, medical records, existence of a dispute between a consumer and a merchant, intentions (for example, to acquire goods or services, or change jobs)."

Secure Document Exchange

We currently use SecureDocs for all document exchange between our clients and health professionals.

SecureDocs classifies data as either file or transactional.  The file (aka attachments) are stored up to a maximium of three months and then irrevocably deleted.  The transactional data is stored for a maximum of seven years and then irrevocably deleted.  Transactional data includes file names, delivery and pickup tracking, and comments.

SecureDocs systems and services are Canadian and all data is stored within Canada under Canadian Law.  They observe and comply with the PIPEDA legislation as well as PHIPA (as an agent to the custodian).
